were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
a) South Side of Dynafusion Structure - Employees working on the outrigger
section of
the HYDRO-MOBILE work platform were exposed to a fall hazard of
approximately
sixty (60) feet, in that appropriate fall protection (ie. end rail system)
was not
implemented.  Observed 06/17/97
ABATEMENT NOTE: One feasible or accepted method of abatement to correct
the hazard is to install the end rail system on the outrigger section of
the
HYDRO-MOBILE work platform.
b) South and East Sides of Dynafusion Structure - The towers of the
HYDRO-MOBILE
work platform were not supported on firm foundation as per manufacturer's
requirements
exposing employees to a fall hazard of approximately sixty (60) feet.
Observed
06/17/97.
ABATEMENT NOTE:  One feasible or accepted method of abatement to correct
the
hazard is to follow the manufacturer's requirements and guidelines for the
installation of
the foot pads (support blocks).
