Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a) South Side of Dynafusion Structure - Employees working on the outrigger section of the HYDRO-MOBILE work platform were exposed to a fall hazard of approximately sixty (60) feet, in that appropriate fall protection (ie. end rail system) was not implemented. Observed 06/17/97 ABATEMENT NOTE: One feasible or accepted method of abatement to correct the hazard is to install the end rail system on the outrigger section of the HYDRO-MOBILE work platform. b) South and East Sides of Dynafusion Structure - The towers of the HYDRO-MOBILE work platform were not supported on firm foundation as per manufacturer's requirements exposing employees to a fall hazard of approximately sixty (60) feet. Observed 06/17/97. ABATEMENT NOTE: One feasible or accepted method of abatement to correct the hazard is to follow the manufacturer's requirements and guidelines for the installation of the foot pads (support blocks). NOTE: ABATEMENT DOCUMENTATION REQUIREDATION

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazards: a) East Side of the Dynafusion Structure - Employee was operating the Pettibone level reach (model # 8042) on uneven ground without the use of the supplied seatbelt. Observed 06/17/97. ABATEMENT NOTE: One feasible or accepted method of abatement to correct the hazard is to implement the use of the supplied seatbelt when operating the level reach.
