Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crush and fall hazards:
a) Manoa Shopping Center, Northeast Corner - Employees were exposed to
crush
and
fall hazards.  Employees working up on the bar joists were exposed to the
likely collapse
of the bar joists due to concentrated loads of bundled roof decking being
placed on the
east end of the bar joists prior to the full and complete installation of
all recommended
bridging (horizontal bracing) components.  Observed 11/12/97.
(On 11/12/1997, two employees were up working on the bar joists installing
bridging.
The employees fell approximately 23 feet when the bars joists on which
they were
working collapsed.  The joists which collapsed, did not have the
recommended bridging
installed prior to the placement of concentrated loads (bundles of roof
decking).
ABATEMENT NOTE:  Among other methods, one feasible and acceptable means of
abatement is to completely install all appropriate bridging prior to the
placement of any
concentrated loads, such as bundles of roof decking.  And also not to
exceed the rated
load capacity of the bar joists as per the Steel Joist Institute
recommendations for erection
of open web steel bar joists.

a) Manoa Shopping Center, Interior, West Side - Employee(s) was exposed to
crush
and fall hazards in that he was operating an elevated scissor lift in an
area that
contained uncovered/unguarded floor hole(s).  On 12/11/1997 an employee
who was
operating the MX15/19 Upright scissor lift while being elevated
approximately 13 feet
above the floor, entered an uncovered/unguarded floor hole causing the
unit (and the
employee in it) to tip over.  The employee sustained serious injury.
Observed
12/11/97.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable means of
abatement is to have all floor holes appropriately covered or to maintain
a safe
distance from obstacles, debris, drop-offs, HOLES, depressions, ramps, and
other
hazards to ensure safe elevated travel as per ANSI A92.6-1990 section
8.10.15.
