Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees
in that employees
were exposed to hazards of fire and explosion at Pad C. The following
conditions contributed
to the existence of this hazard:
a)  The inert gas system was not equipped with a means to allow operations
or
maintenance personnel to monitor the status or condition of the system on
an individual
vessel or tank. Operating procedures had not been developed for the
operation of the
inert gas system servicing the Pad C solvent tanks.  The standard
operating procedures
for the recycling of Tank 219 was incomplete, in that it did not address
temporary
conditions such as extended recycle times that may result from
environmental,
production, or other causes.  Piping inspection program for flammable and
combustible
liquids piping at the Pad C area was inadequate.
Abatement Note:Feasible abatement for this item would include the
following.
Other abatement steps may also be acceptable.
Develop and implement the means to allow operations and
maintenance personnel to monitor the status of the inert gas system
on individual tanks.
Procedures and information addressing the operation of the inert
gas system should include how the system functions at Pad C
including a description of all system components; operating
parameters; consequences of deviation from specifications; and
steps to avoid or correct deviations.
Procedures should be developed for recycle times for Tank 219 not
addressed by operating instruction in effect on January 2nd, 1999.
Procedures should be developed for reviewing delays in completion
and implementation of Corrective Action Report items.
A piping inspection program should be developed and implemented
in accordance with API 570 specifications.
