Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to fall and crush hazards:
a) Exton Square Mall, Overbuild Area - Employees exposed to
the likely collapse of the bar joists due to concentrated
loads of bundled steel roof decking being placed on the ends
of the joists prior to the complete installation of all
recommended bridging (horizontal bracing) components.
ABATEMENT NOTE:  Among other methods, one feasible means of
abatement is to completely install all appropriate bridging
prior to the placement of any concentrated loads, such as
bundles of roof decking, as per the Steel Joist Institute's
recommendation which was located on the field drawings.
NOTE: ABATEMENT CERTIFICATION REQUIREDATION
