employer
did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing injuries:
a) Sonco Worldwide - Employees in the shipping/receiving yard did not wear
seatbelts
when operating the Toyota forklifts.  The employees operated the forklifts
on terrain with
several large "potholes", including one that measured 4.5 feet long by
3.75 feet wide by
approximately 8 inches deep (outside of the warehouse door).
ABATEMENT NOTE:   One feasible means of abatement would be to require
employees to utilize the appropriate restraint device that is provided.
