Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of electrical shock, trauma or burns where exposure to the potential use of ungrounded appliances in the presence of water or in a potentially wet/damp location. The following condition contributed to the existence of this hazard: (a)P/V Arctic Star, room #12 - The electrical outlet located above the wash basin/sink was not a GFCI protected outlet nor was the circuit protected by a GFCI breaker at the panel. One feasible means of abatement, among others, would be to replace the regular outlet with a GFCI type outlet.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of electrical shock, trauma or burns. The following conditions contributed to the existence of this hazard: (a)P/V Arctic, engine room - Where an extension cord (two wire type with ground pin) was in use to power a Graymill parts washer and the cord indicated an open ground in the wiring when tested with IDEAL circuit tester #61-035, the employer failed to ensure that the extension cord was in a safe and serviceable condition prior to allowing it to be used as a power source. One feasible means of abatement, among others, would be to inspect the extension cord prior to its use and repair or destroy and discard the defective equipment.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of a falling load while using lifting equipment which contained a defective safety mechanism and the defective gear for rigging and materials handling was not removed and repaired or replaced before further use. The following condition contributed to the existence of this hazard: (a)P/V Arctic star, in the battery change out/charging area - Where the chain hoist hook used to lift the forklift batteries contained a bent/sprung safety latch which did not effectively bridge the throat opening. The employer failed to inspect the equipment before use and failed to lockout/tagout, remove from service or repair the defective equipment before further use. One feasible means of abatement, among others, would be to replace the defective safety latch with a new one.
