Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a) M/V Woodbine: The power panel in the employee breakroom was missing
circuit
breaker #19 and energized wiring/terminals were visibile through the
opening. This condition
exposed employees, who had occassion to access the panel, to the hazard of
electric shock, burns
and/or electrocution. Among other methods, one feasible and useful method
of abatement would
be to install a cover blank in the unused opening.
b) M/V Woodbine: The electrical outlet, adjacent to the power panel in the
breakroom,
indicated reversed polarity when tested with IDEAL circuit tester 61-035.
This condition exposed
the employees, who had occassion to use this outlet, to the hazard of
electric
shock or burns and
electrocution. Among other methods, one feasible and useful method of
abatement would be to
reverse the wiring at the receptacle.
c) M/V Woodbine: In the womens bunkroom, two electrical outlets indicated
reversed
polarity when tested. This condition exposed employees, who had occassion
to use this outlet,
to the potential hazard of electrical shock or burns and electrocution.
One feasible and useful
method of abatement would be to correct the wiring inside the outlet.
d) M/V Woodbine: In the womens bunkroom, one electrical out indicated an
open ground
when tested. This condition exposed employees, who had occassion to use
this outlet, to the
potential hazard of electrical shock, burns and/or electrocution. One
feasible and useful method
of abatement would be to reconnect the ground wire in the outlet.
e) M/V Woodbine: In the box storage area, aft of the forward deck, one
electrical outlet
indicated an open ground when tested. This condition subjected the
employees, who had
occassion to use this outlet, to the potential hazard of electrical shock,
burns and/or
electrocution. One feasible and useful method of abatement would be to
reconnect the ground
wire in the outlet.
f) M/V Woodbine: On the main deck at the box offloading conveyor, the
conveyor motor
had exposed wiring to which the insulation was open to abrasion/cutting
and a wet environment
and subjected employees, who could inadvertently contact the metal
surface, to the potential
hazard of electrical shock, burns and/or electrocution. One feasible and
useful method of
abatement would be to reattach the cover to the motor with a water tight
seal. (abated during
inspection)
