Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the potential hazard of severe electrical shock and or electrocution. The following conditions contributed to the existence of these hazards: a) F/V Mr.B: 110/120 volt electrical outlets located in potentially wet/damp locations near the Galley sinks were not of the Ground Fault Circuit Interrupter (GFCI) type and the controlling branch circuit was not equipped with a GFCI type circuit breaker at the panel. One feasible means of abatement, among others, is to install a GFCI type circuit breaker in the control panel. b) F/V Mr.B: In the galley; a 110-120 volt duplex outlet was missing its coverplate and energized wiring and terminal screws were visible and accessible. One feasible means of abatement, among others, would be to replace the coverplate. c) F/V Mr.B: In the cooks prep area, two each 110-120 volt outlets indicated reversed polarity when tested with IDEAL circuit tester 61-035. One feasible means of abatement, among others, would be to rewire the receptacle correctly. d) F/V Mr.B: In the engine room, the power panel, behind the eyewash unit, was missing the main breaker and energized wiring was visible/accessible through the opening. One feasible means of abatement, among others, would be to insert a blank metal cover into the opening. e) F/V Mr.B: In the machine shop, a large power panel was missing the entire inside cover and all energized wiring, terminal screws and busbars were visible/accessible to touch. One feasible means of abatement, among others, would be to replace the inside cover. f) F/V Mr.B: In the laundry room, a 110-120 volt outlet was missing the coverplate and energized wiring and terminal screws were visible and accessible. One feasible means of abatement, among others, would be to replace the coverplate. Note: The South Atlantic Fisheries, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard under section 5(a)(1) which was contained in OSHA Inspection Number 301494571, Citation Number 01, Item Number 001, issued on 04/27/98, with respect to a workplace located on the F/V Mr."B". in Dutch Harbor, Alaska.
