Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a.)P/B ARCTIC STAR: The cover was missing from the thermostat in the operator's cab of the starboard crane exposing the live wiring. Note: Among various feasible and economical abatement methods, one would be to replace the missing cover. b.)P/B ARCTIC STAR: There were two circuit breakers missing from the reefer van electrical panel in the forepeak area, exposing the open buss bar. Note: Among various feasible and economical abatement methods, one would be to install blanks in the unused openings.
