Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the potential hazard of severe electrical shock and/or
burns. The following
conditions led to the existence of these hazards:
a)P/B Alaskan Venturer: Where two metal clad box fans were in use as Roe
dryers, and
where these fans were located in a potentially wet/damp location in the
Roe processing
room, the exposed non-current carrying metal parts of the fans were not
grounded. This
condition exposed employees to a potential hazard of severe electrical
shock and/or burns
during inadvertent contact with the energized outer casing of the fans
while standing on
a damp metal deck.
Note: Among other methods, one feasible and effective method of abatement
would
be to
purchase an industrial type fan that contains an equipment grounding
conductor.
b)P/B Alaskan Venturer: In the Roe room, an extension cord equipped with
two additional
adapters was in use as a permanent power source for the drying fans. This
condition
exposed employees to a potential hazard of severe electrical shock and/or
burns while
handling these adapters with wet hands and while standing on a wet/damp
metal deck.
Note: Among other methods, one feasible and effective method of abatement
would be to install
additional receptacles where needed to eliminate the use of the cord.
c)P/B Alaskan Venturer: Where the extension cord was installed in a
wet/damp location,
it was not suitable for the location in that it was not designed for use
in a wet/damp
environment and was not equipped with a waterproof cover. This condition
exposed
employees to a potential hazard of severe electrical shock and/or burns
while handling
the cord in a wet/damp environment.
Note: Among other methods, one feasible and effective method of abatement
would be to
remove the extension cord and install permanent receptacles where needed.
d)P/B Alaskan Venturer: On the starboard weather deck, where the 220 volt
power cord
to the welder had been spliced, the repaired section was only covered with
electricians
tape which did not retain the insulation, outer sheath properties, and
usage characteristics
of the cord in its original condition.  This condition exposed employees
to a potential
hazard of severe electrical shock and/or burns when handling the cord
while standing on
a wet/damp metal deck.
Note: Among other methods, one feasible and effective method of abatement
would be to shorten
the cord by disconnecting the splice and installing a connector plug on
the
service end of the
cord.ce
