Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
a)Inpatient Pharmacy:  On or about January 15, 2002, and times prior
thereto, the Class
II, Type B1, Biological Safety Cabinet (BSC) was not serviced and
certified every six
months.  It was noted at the time of this inspection that the BSC was last
certified on
May 09, 2001 and was due for certification on November 01, 2001.
b)Inpatient Pharmacy and Oncology staff: On or about January 15, 2002, and
times
prior thereto, employees prepared and administered chemotherapy and were
not
provided with medical surveillance including, but not limited to, periodic
medical
examinations.

correct this hazard
is to follow the guidelines established by the American Society of
Hospital Pharmacists
(ASHP), 1990, ASHP Technical Assistance Bulletin on Handling Cytotoxic
Drugs.  Am J.

Antineoplastic-Drug
Handlers is described in the Am. J. Hosp. Pharm. 1990; 47: 1061-66.
