Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of fire and/or explosion:
a)King Salmon Platform, well # K-17:   On or about April 20, 2002, and at
times prior
there to, well control was not maintained at all times as required by
American Petroleum
Institute API 54 Third Edition, August 1999 Recommended Practice for
Occupational
Safety for Oil and Gas Well Drilling and Servicing Operations, Section
6.1.1, and Alaska
Oil and Gas Conservation Commission Regulation 20 AAC 25.033(a).  While
performing
ESP well work-over procedures, a well control event occurred that resulted
in an
uncontrolled release of gas and liquid hydrocarbons (oil). The
hydrocarbons ignited
resulting in a fire and injuries to personnel.
Feasible and acceptable abatement methods among others to correct this
hazard is to establish
a well control safety program and procedures for ESP conversion workover
that will be
implement when there are any indications that the well will flow through
prior records, present
well conditions or the planned well work. The program at a minimum shall
contain pertinent
well control procedures contained in American Petroleum Institute
Recommended Practice 59
(RP 59) First Edition, August 20,1987 Recommended Practices for Well
Control Operations,
and Alaska Oil and Gas Conservation Commission Regulations Title 20 -
Chapter 25.  The
employer shall also adhere to the  following requirements on all ESP
work-over operations: (1)
install shear rams on the BOP stacks (2)  conduct well control drills for
all well work-over
crews, and demonstrate the ability to shut the well in through the use of
properly installed and
maintained blowout prevention equipment (3) in known or suspected hydrogen
sulfide
bearing
wells the well control drills shall include employee response to a
hydrogen sulfide gas release.
