Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)P/V Bering Star, Pipe Sponson Area - The 120 VAC yellow extension cord
that ran
through the bulkhead hatch opening was not protected from damage, exposing
employees
to an electrical shock hazard.
NOTE: Among other methods, feasible and effective abatement methods would
be to cover the
extension cord, or remove it from the opening.
(b)P/V Bering Star, Port Crane - The 120 VAC receptacle showed open ground
when tested
with a circuit tester, exposing employees to the hazard of electrical
shock.
NOTE: Among other methods, one feasible and effective abatement method
would be to ground
the electrical receptacle.
