Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire, explosion, burn, and or dermal hazards. (a)Chugach Eareckson Support Services (Shemya Island, AK, outside Environmental Shop): The employer who conducted routine incineration and open flame burning failed to follow the manufacturers safety instructions for the Elastec Smart Burner in that: (1) A flame screen not installed during burning operations. (2) The employer failed to develop standard operating procedures, adequately train employees on operating the burner, and requirements for employees to wear appropriate personal protective equipment. (3) The employer failed to follow manufacturer's guidelines for igniting and starting the unit. (4) The employer was operating the unit unattended in an unfenced area. Note: One feasible and acceptable method of abatement includes following the manufacturer's safety rules located in the Smart Ash Incinerator manual.
