Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/crushing hazards: a)Concord Station - On or about 09/12/06, employees were operating a temporary lift/hoist that was not maintained properly exposing employees to struck-by/crushing hazards. Among other methods, one feasible and acceptable abatement method is to follow ANSI standard MH30.1-2000, For the Safety, Performance, and Testing of Dock Leveling Devices, Section 4.4.5 and ANSI Standard MH29.1-1994, Safety Requirements for Industrial Scissors Lifts, Section 12.3
