Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/crushing hazards: a)P14 Elevator, VA Hospital, Memphis, TN - On or about 02/24/09, employees were exposed to struck-by/crushing hazards when proper communication procedures were not followed. This hazard was reasonably foreseeable in that it was recognized by employer representatives. Among other methods, one feasible and acceptable method to correct this hazard would be to follow company policy for communication between floors and retrain employees on existing work rules for communication.
