Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
The hazard associated with improperly guarded press brakes which include
crushing and
amputation.  The two hand controls were not properly set up on the press.
The controls did not
require concurrent pressure, were not protected against double tripping
and the buttons were not
protected against accidental cycling.  The E-stop button was not red in
color and frequent
inspections of the press and of all of the associated safety equipment was
not conducted as
required by A.N.S.I. B11-3 1973.
One feasible and acceptable method to abate this condition is to redesign
the two hand
control system so they require concurrent pressure, are protected by rings
around
the
buttons and incorporate an anti-repeat feature, replace the E-stop with a
red mushroom
button and train all employees in the safe operation of the machine.  Set
up a program
of frequent inspections of the press and all associated safety equipment
and maintain
records of these inspections.
