In Plant 5, T-501, Serial Number C-2010, constructed for a pressure of 43
psi had had
safety relief valves RV-502A and RV-502B.  These valves were manufactured
by
Consolidated, Model No. 1906-RC-1, Serial Numbers TJ31777 and TJ56057.
The set
pressure for both of these relief valves was 45 psi.  Operators working in
the area were
thereby exposed to the hazards associated with having pressure relef
valves set at an
operating pressure in excess of the maximum allowable working pressure for
a pressure
vessel.
Feasible methods of abatement include, but are not limited to, the
following:
1)Select and install pressure relief valves that meet the requirements of
Section VIII of
the American Society of Mechanical Engineers ((ASME) Pressure Vessel Code.
2) Re-rate the maximum allowable working pressure of the pressure vessel
in excess of
the set pressures of the pressure relief valves as set forth in Section
VIII of the ASME
Pressure Vessel Code.
