Section 5a()(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or were likely to cause death or serious physical harm  to
employees in that
employees were exposed to:
A contract employee working for Bowen Arrow, reported to Worsham Steel Co,
located at
14001 Montana Ave, El Paso,Tx on or about April 6,1998 to check on an
overhead bridge crane
that was reported to Bowen Arrow as not operating properly. The Bowen
Arrow employee
working with Worsham Steel foreman and forklift operator, attached a metal
cage to a forklift
and raised the Bowen Arrow Employee up to the Bridge crane to check on the
crane's brake
system.
While the Bowen Arrow employee was working approximately 30 feet above
ground level, the
forklift operator left the forklift in the raise position with the worker
still on the man lift cage.
This condition exposed the employee on the cage to fall hazards and caught
between hazards.
Feasible and acceptable methods to correct this hazard include, but are
not limited to the
following:
a. Establish written procedures covered in ANSI B56.1-1975, which states,
there shall be an
operator in the control position on the truck.
b. Implement employee training on company written procedures
