Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
a)The employer used and charged an anhydrous ammonia refrigeration system
with
farm-grade anhydrous ammonia, which contained as much as 2water, instead
of
refrigeration-grade ammonia, which ANSI/IIAR 2-1999 (Equipment, Design, and
Installation of Ammonia Mechanical Refrigerating Systems) states shall
have a
maximum of 33 ppm water content.  The water content can form ice in the
lines
and block ammonia flow in the system, which can lead to a rupture in an
ammonia
containing line.  An abatement method would have been to remove water from
the
lines and to charge the system with refrigeration-grade
ammoniaration-grade
