Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that
employees were
routinely exposed to the hazards of serious burns from contact with steam
and hot condensate
while in steam pits due to failure to adequately maintain and inspect the
steam distribution
system  and failure to adequately train repairer/operator personnel.
Steam Distribution System - During routine seasonal activation of the base
steam system
on 10/21/97 a civilian employee was seriously burned while in steam pit
D-3 due to the
rupture of a 10" steam valve.
Among other methods one feasible and acceptable abatement method to
correct this
hazard is to implement a maintenance and inspection program based on such
documents
as NAVFAC MO-209 and manufactures equipment recommendations and to
implement
a training program to teach repairer/operator personnel to recognize
potential hazards,
and defects such as that condition existing in steam pit E-1 whereby the
expansion joint
has moved from it's supporting foundation.

steam distribution system.
Steam Pit D-3 - Condensate drains (mud legs) in this pit were undersized
for the 14"
steam line.
Among other methods one feasible and acceptable abatement method is to
size the drains
in accordance with the ASHRAE HANDBOOK and the Piping Handbook (min. 4")..
