Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to over-pressure hazards of pressure vessels: At the Plant - The following pressure vessels did not have over-pressure protection: a)The Extractor b)The Process Tank Abatement note: A feasible means of abatement is to install safety relief valves on these pressure vessels. Section VIII of the ASME (American Society of Mechanical Engineers) Boiler and Pressure Vessel Code requires safety relief valves on pressure vessels for over-pressure protection. As an added precaution, the process controls on the extractor must be operated in the "automatic" position, and locked in that position so the "hand" or "manual" position cannot be used. In the automatic operating position, the instrumentation activated the high pressure set point of 210 psi, limiting the operating pressure to 210 psi under normal operating conditions. In the manual position the high pressure set point was bypassed, and the potential pressure was 360 psi, the rated pressure of the centrifugal water pump. This exceeded 250 psi, the maximum allowable working pressure of the extractor vessel. Another possible precaution was to lock open the bypass on the water pump to the extractor so that the pump pressure is maintained below the 250 psi rated pressure of the extractor vessel.or
