Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing
or likely to cause death or physical harm to employees in that employees
were exposed to:

El patrono no proveyo
empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que puedan causar
muerte o dano fisico a los empleados, por cuanto empleados estaban
expuestos a:
Exposed to a falling hazard at SEDECO SERVICIOS DE DESCUENTO EN COMPRA,
INC.
located at Sector Industrial Sabana Garden Lote 13B Carolina PR  00987, in
two occasions  an
elevated employee without the benefit of a fall arrest system, was
observed moving merchandise
from a bin to a dolly, with a risk of falling thru open spaces,.  The same
employee that moved the
merchandise operated the picker and elevated the dolly to the desired
height and position.   Once the
dolly was positioned the transfer of merchandise from the bin to the dolly
resumed as follow:
An employee was observed moving merchandise while elevated six (6) feet
from the floor
at the rack number seventeen  (17).  The elevated dolly was 43 inches
wide, while the
distance between racks was of 69 inches.  On one side, where the
merchandise was being
transferred, there was an open space between the rack and the bin of
fifteen (15) inches.  On
the other side of the dolly there was an open space of eleven (11) inches.
b) An employee was observed moving merchandise while elevated twelve (12)
feet from the
floor at the rack number nine  (9).  The elevated dolly was 43 inches
wide, while the distance
between racks was of sixty five (65) inches.  On one side, where the
merchandise was being
transferred, there was an open space between the rack and the bin of
twelve (12)  inches.  On the
other side of the dolly there was an open space of ten (10) inches.
Among other methods one feasible and acceptable abatement method to
correct this hazard  is:
1. placing the racks in such a manner that the hall between them has a
width
that
reduces the gap between the racks and the dolly; or
2. changing the 43 inches wide dollys for a wider one; or
3. modify the dollys so that their platform is wider.
Con un riesgo de caida en  SEDECO SERVICIOS DE DESCUENTO EN COMPRA, INC.
localizado en Sector Industrial Sabana Garden Lote 13B Carolina PR  00987,
en dos
ocasiones un empleado elevado, sin el beneficio de un sitema de proteccion
contra caidas,
fue observado moviendo mercancia de un bin a un carreton, con el riesgo de
caer por un
espacio abierto.  El mismo empleado que movia la mercancia operaba el
picker y elevaba
el carreton a una altura deseada y possicion.  Una vez el carreton estaba
colocado la
transferencia de mercancia del bin al carretoon comensaba como sigue:
Se  observo a un empleado moviendo mercancia mientras estaba elevado a
seis (6) pies desde el piso en el rack diesisiete (17).  El carreton
elevado tenia
43 pulgadas de ancho, mientras que la distancia entre los racks era de 69
pulgadas.  En un lado habia una apertura entre el rack y el carreton de 15
pulgdas.  En el otro lado del carreton habia una apertura de once (11)
pulgadas.
Se  observo a un empleado moviendo mercancia mientras estaba elevado a
quince (15) pies desde el piso en el rack nueve (9).  El carreton elevado
tenia43 pulgadas de ancho, mientras que la distancia entre los racks es de
65
pulgadas.  En un lado habia una apertura entre el rack y el carreton de 12
pulgdas.  En el otro lado del carreton habia una apertura de 10 pulgadas.
Entre otros metodos de correccion metodos factibles y aceptables de
correccion son:
colocar los anaqueles de forma que se reduzca el espacio entre los racks y
el
carreton; o
cambiar los carretones de 43 pulgadas de ancho por unos mas anchos;
modificar los carretones de forma que tengan una plataforma mas ancha.

Presenting the hazard of stricking employees at SEDECO SERVICIOS DE
DESCUENTO EN COMPRA, INC.
located at Sector Industrial Sabana Garden Lote 13B Carolina PR  00987,
where the employees drove fingers,
drove pickers, stacked merchandise, and performed other tasks, the rack
next to the rolling door number 7, where
microwaves, hoods and other merchandise was stored, had  four (4) of its
frontal seven (7) legs damaged.  This
rack's legs where damaged by fingers and pickers operated by employees
while stacking merchandise.  This
structure had 50 feet long, 4 feet wide and 15 feet tall.  The rack was
not anchored to the wall.  The rack had three
storing areas: the floor storing area, the middle area, and the upper (15
feet  above ground) area.  On the upper
storing area there were microwaves.  This structure integrity was
compromised.
Among other methods one feasible and acceptable abatement method to
correct this hazard is
replacing the rack.
Creando el riesgo de empleados ser golpeados, en SEDECO SERVICIOS DE
DESCUENTO EN
COMPRA, INC. localizado en Sector Industrial Sabana Garden Lote 13B
Carolina PR  00987, donde los
empleados guiaban fingers, guiaban pickers, guardaban mercancia, y
realizaban otras tareas, el rack
al lado de la puerta numero 7,  donde habian microhondas, extractores y
otra mercancia almacenada,
un rack tenia cuatro (4) de sus siete (7) patas delanteras danadas.  Las
patas del rack fueron danadas
por montacargas y pickers operados por empleados mientras estos
almacenaban mercancia.  Esta
estructura tenia 50 pies de largo, 4 pies de ancho y 15 pies de alto.  El
rack no estaba anclado a la pared.
El rack tenia tres areas de almacenado:  el area de almacenado a nivel de
piso, el area del medio, y el
area superior (que estaba a 15 pies sobre el piso).  En el area de
almacenaje superior estaban los
microhondas.  La integridad de la estructura estaba comprometida.
Entre otros metodos de correccion uno factible y aceptable de correccion
es reemplazando el rack.
