Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:
Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico:
El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
The hazard is employees being struck by the load.  The dead end side of
the crane wire
rope on a wedge socket type termination at the headache ball was attached
to the live line
with a wire rope clip.  The clip will transfer the load to the dead end
side of the wire
rope causing damage to the live side of the wire rope and/or cause
instability in the crane
hoist system and load.  Employees were placing and removing form work
molds from
the second floor, of the Southeast, type A building of the Costamar Beach
Village West,
located on Municipal Street, Mediania Alta, Loiza, with a crane, make
Lima, model
700TC, serial number 4A0189490, type motor crane, with a capacity of 75
tons, located
at the rear of the building.  Among other methods, one feasible and
acceptable abatement
method to correct is to remove the wire clip from the live line and place
it on the dead
end side, check to see if the live line has been damaged, and if so,
replace the wire rope.
El riesgo es que los empleados sean impactados por la carga.  En la
terminacion de
casquillo de cuna, ubicado en el gancho con peso de la grua, el cable
terminal que sobre
sale estaba amarrado junto a la linea principal del cable de alambre de la
grua con una
presilla para cable de alambre.  El efecto es que la presilla transfiere
la carga al cable
terminal causando dano a la  linea principal y o causando inestabilidad en
el sistema de
izar de la grua y en la carga.  Los empleados estaban instalando y
removiendo los
moldes de las formaletas del segundo piso, del edificio tipo A del lado
sur
del proyecto
Costamar Beach Village West, ubicado en la calle Municipal, Mediania Alta,
Loiza, con
una grua, marca Lima, modelo 700TC, numero de serie 4A189490, tipo grua de
motor,
con capacidad de 75 toneladas, ubicada en la parte trasera del edificio.
Entre otros
metodos aceptables para corregir, uno viable y aceptable es remover la
presilla y
reponerla en el cable terminal, revisar por dano en la linea principal, y
si hay dano
reemplazar el cable de alambre.
