Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing
or likely to cause death or physical harm to employees in that employees
were exposed to:

empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que puedan causar
muerte o dano fisico a los empleados, por cuanto empleados estaban
expuestos a:
At Corredor del Este AC 006621 project  located at Bo Martin Gonzalez
Carretera Ramal 860 Km
2.0 Carolina, a Prima Facie, Inc. employee was observed misusing or
failling to follow the safety
rules stated in the instruction manual of the equipment when he was
observed  polishing a steel
angular with a portable  angle grinder  holding the the rear handle of the
angle grinder with his right
hand and the steel angular with his left hand while the spinning disk was
facing
upward.. The
equipment instructions manual state the following on page 9:
Grinding and sanding operation
Always hold the tool firmly with one hand on rear handle and the other on
the side handle.
Turn the tool on and then applly the wheel or disc to the workpiece.
The angle grinder used by the employee was Makita trademark.  This grinder
could be a 7" Model
9067.
Hazard: Eyes, face, hands, arms, and body of the employee exposed to
shattered wheel shards flying
at high speed.
Among other methods one feasible and acceptable abatement method to
correct this hazard is
providing a well mounted and fixed bench grinder.
En el Proyecto Corredor del Este AC 006621, localizado en el Bo Martin
Gonzalez Carretera Ramal
860 Km 2.0 Carolina, un empleado de Prima Facie, Inc. fue observado
utilizando
mal o no siguiendo
las reglas de seguridad del manual de instrucciones del equipo cuando el
fue observado puliendo un
angular de metal con una pulidora portatil agarrando el mango trasero de
la pulidora portatil con su
mano derecha y el angular de metal con su mano izquierda con el disco
dando vueltas y mirando
hacia arriba.  El manual instrucciones del equipo indica lo siguiente en
la pagina 9:
Grinding and sanding operation

La pulidora portatil que usaba el empleado era marca Makita.  Esta
pulidora pudo ser una de 7"
modelo 9067.
Riesgo: Ojos, cara, manos, brazos, y cuerpo del empleado expuesto a
pedazos de la rueda de pulir
destrozada volando a altas velocidas.
Entre otros metodos de correccion uno factible y aceptable de correccion
es
proveer una pulidora de
mesa bien montada y fijada.
