A.  Employer knew or should have known that one employee operating Reactor
No. 1 in the
second phase pilot laboratory on April 29, 1998 which was not equipped
with any safety
relief devices were exposed to the hazards of chemical releases, fire,
explosions and being
struck by the reactor.  A feasible and useful method to correct these
hazards,
among other
methods, is to install pressure relief devices such as, but not limited
to, pressure relief valves
and rupture discs on the reactor in accordance with ASME code.
