SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that miscellaneous construction employees required to install a 6' x 6' x 12' catch basin for french drain project at slurry pond #2 were exposed to the hazard of an unknown atmosphere. A feasible and useful method to correct this hazard, among other methods, is to evaluate the workplace for confined spaces.

SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that miscellaneous construction employees working in and around the 6' x 6' x 12' catch basin for the french drain project next to slurry pond #2 at Winyah Generating Station were exposed to the hazard of an unknown atmosphere with high concentrations of hydrogen sulfide and sulfur dioxide and oxygen deficiency. A feasible and useful method to correct this hazard, among other methods, is to implement a confined space program for miscellaneous construction employees.

Employer knew or should have known that Winyah Generating Station employees performing rescue in a 6' x 6' x 12' catch basin without using a body harness with retrieval line or self contained breathing apparatus (SCBA) were exposed to the hazard of an unknown atmosphere. A feasible and useful method, among other methods, is to require employees performing confined space rescue to wear a body harness with retrieval line or wear a SCBA.

Employer knew or should have known that miscellaneous construction employees riding in the bucket of the John Deere backhoe to enter a 6' x 6' x 12' catch basin at slurry pond #2 at Winyah Generating Station were exposed to the hazard of falling into an unknown atmosphere. A feasible and useful method to correct this hazard, among others, is to provide ladders or other suitable means of descent into catch basin.

Employer knew or should have that ont employee from the Winyah Generating Station performing rescue on an employee in the 6' x 6' x 12' catch basin for the french drain project at the Winyah Generating Station was exposed to the hazard of an unknown atmosphere. A feasible and useful method to correct this hazard, among other methods, is to implement confined space rescue procedures to include: 1) prevention of unauthorized personnel from attempting rescue 2) hazard recognition training for all employees who perform confined space rescue 3) the use of respiratory equipment such as supplied air respirators with an escape bottle or self contained breathing apparatus (SCBA).us
