each of his employees condition(s) of employment and place(s) of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm in that
employees were exposed to potential hazards related to hoist
failure and falling suspended loads.
a)Plant, pipe cutting area - Acco-Wright-Way (1/2 ton/
120/240 vac/ pendant control) wall mounted swing jib
underhung chain hoist, chain and hook assembly were not
routinely inspected, tested and records maintained in
accordance with American National Standards Institute
safety standards for cranes, hoists and slings (ANSI
B30.16/ ASME 1981).

b)Plant, welding area -  5 Jet (1/2 ton, 120/240 vac/
pendant control) suspended chain hoists were not
routinely inspected, tested and records maintained in
accordance with ANSI B30.16/ ASME 1981.

development and implementation of inspection procedures and
records maintenance in accordance with the machine
manufacturer's documentation manual and ANSI B30.16/ ASME
1981)./
