North Carolina General Statue 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees  condition of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees in that employees were exposed to:
a)a fall down an open elevator shaft, in that prior to the
elevator's connection to call buttons, the employees were
instructed to open closed elevator hoistway doors with a
hoistway door key to access the elevator car, without
knowledge whether the elevator car was in the shaft behind the
doors.
Among other methods, one feasible and acceptable method to correct
this hazard is for the operator to park the elevator car at the
lowest level, i.e., basement level, and either put the doors in a
"locked" open position whenever the operator is not in the car or
have the supervisor in control of the hoistway door key open the
hoistway doors at the beginning of operating the elevator.

Section 111.  Rule 111.9e(4): The operating means for unlocking the
door shall be available to and used only inspectors, maintenance
men, and repairmen.e
