Title 40.1-51.1.(A), Code of Virginia: The employer did not furnish employment and a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees in that: Located in the cake hopper area, where employees adjust the centrifuge cake feed, the cake bypass, and the cake hopper levels and perform maintenance and housekeeping duties, the employees were exposed to an explosion and fire hazard on June 4, 1998, when a Lower Explosive Level (LEL) of 11was measured at the cake bypass switch, tank level controller, and diverter gate electrical panels. One feasible and acceptable abatement method, among others, to correct this hazard would be to eliminate the combustible gas source and provide adequate ventilation and an alarm device in the area that would warn employees working in the area or warn them prior to entering the area.
