Code of Virginia 40.1-51.1.(a): The employer, Maco Tool, Inc., did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a flying part (bronze adaptor and fixture) while operating a lathe with a Pratt Burnerd setrite chuck (Model 9219- 00370) in excess of the manufacturer's gripping range on May 28, 1998. One feasible and acceptable abatement methods, among others, to correct this hazard is to supply and enforce the use of holding equipment that is of adequate size, capacity and condition to perform the type of operation required on the lathe as that called for by ANSI B11.6-1984, American National Standard Safety Code for Machine Tools and Lathes.
