Title 40.1-51.1,(A),Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that: (a) The vertical lifeline that was being used as part of a fall arrest system for the employee working from the boatswain's chair at the front of the 3 story building lacked 12 feet of reaching the ground. One feasible and acceptable method among others to correct this hazard is to adhere to The Occupation Safety and Health Standards for The Construction Industry (29 CFR Part 1926), Subpart M, Part 1926. 502(d)(16)(iii): Fall arrest systems when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet nor contact any lower level.
