1910.147(c)(4)(i): At the laminator on press #1, procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in servicing or maintenance operations such as washing/cleaning and lubricating the laminator rolls. On or about 03/19/1999, an employee was injured after becoming caught in an in- running nip point during a power on cleaning operation of the laminator on press # 1.
