North Carolina General Statute 95-129 (1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: facility wide - cranes were not load tested prior to initial use such as: a)Hyster Battery Shop - 2 Ton Underhung Crane which was rated at 2 Ton capacity and is lifting batteries weighing 3,734 lbs. b)Metal Department #33 - 3 Ton Underhung Crane which is used to move coil stock. c)Machine Shop - 2 Ton Underhung Crane used for lifting and moving machine parts. Reference:ANSI B30.11-1980; paragraph 11-2.2.2 (a) Rated Load Test NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to load test all cranes as per ANSI B30.11-1980.

North Carolina General Statute 95-129 (1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)facility; battery hyster shop & loading dock - below the hook lifting devices such as the one used in the battery hyster shop to move batteries, and the one on the loading dock used to unload trailers were not inspected prior to initial use, frequently and periodically. REFERENCE:ANSI B30.20-1985 Section 20-1.3 - INSPECTIONS Prior to initial use, all new, altered, modified, or repaired lifting devices shall be inspected by a designated person. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to have all below the hook lifting devices inspected, and continue on a frequent and periodic basis.

North Carolina General Statute 95-129 (1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)facility; throughout - cranes such as the 2 Ton Underhung crane which supports a chain driven Coffing Hoist used in the machine shop area to move parts, was not marked with the Rated Load. REFERENCE:ANSI B30.11 Section 11-1.1.1 Rated Load Markings - Crane The rated load of the crane shall be marked on the crane. RATED LOAD is defined by ANSI as: The maximum load, designated by the manufacture or qualified person, for which the crane or monorail system is designed and built. NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to have all cranes load tested and display the Rated Load on the Crane Bridge.

North Carolina General Statute 95-129 (1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)facility; machine shop area - 2 Ton Underhung Crane supporting a chain driven Coffing Hoist was not equipped with a stop to prevent the crane from making direct contact with the supporting structure. REFERENCE:ANSI B30.11-1.3.1 Stops shall be provided at the ends of the carrier or crane travel to prevent the carrier or crane from making contact. Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install stops on the ends of the crane.

NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to have all below the hook lifting devices marked with the rated load.
