Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employees working in the decking department in the vicinity of the
underhung crane on aisle
3, were exposed to hazards of falling loads, falling hoists, and/or hoist
components due to the
following conditions:
a) the load capacity for the crane had not been detemined with a load test
performed following
crane installation but prior to service use.
b) the hoist jumped on the runway as it passed the worn out spots in the
bridge.
c) the load capacity of the crane/hoist was not marked on each side of the
hoist or bridge.
One feasible and acceptable method, among others, to correct these hazards
is:
Installation, testing and establishment of inspection and maintenance
procedures, which include
conducting frequent and periodic inspections by a competent person on the
crane and hoist,
including components and structure and to take corrective measures as
needed in accordnace with
American National Standards Institute B30.17 - 1992 Safety Requirments for
Overhead and
Gantry Cranes Top Running Bridge, Single Girder, Underhung Hoist.
