Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to potential burns from fuel gas where cracked 3 inch diameter flexible tubing was repaired with black electrical tape in #1 & #3 Finishing the Anneal Department. Furnaces have natural gas input pressure of 25 psi and electrical tape was not designed for this purpose. Among other feasible methods one way to correct this hazard is: 1. for repairs, follow the ANSI B31.2 - 1968 Fuel Gas Piping 223.1.5 which states: " materials which do not conform to any standards or specifications listed herein, nor to any standards or specifications listed in USAS ANSI! B31.3, shall be used only after they have been approved for the intended use by USA ANSI! Standards Committee B31 under USASI ANSI! procedures." 2. Remove the gas from the cracked piping and replace cracked flexible piping rather than repairing the short flexible connector pieces. 3. Train on and enforce the above procedures.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to impact injuries from movement of coils from; A. loading and operation of coil car "buggies" in #1 & #3 Finishing Anneal Department where emergency stops on the sides of cars did not operate and total coil weight limitations were exceeded. Among other methods, the following are feasible and acceptable methods to minimize the probability of injury during coil car operations: (1) Ensure coil transfer cars are operated within weight capacity and are maintained in good repair by; a) ensuring frequent visual inspection and testing of operational controls and safety stops, and b) prohibiting overloading of coil cars. (2) Ensure those participating in coil car loading activities are trained in the limitations of the coil cars and proper loading procedures. (3) Enforce weight capacity limitations of the coil cars per LTV Procedures JSAAN 11 and JSAAN 18 which state: "never overload buggies". B. unexpected start-ups of the coil conveyor in the DHCC Warehouse where audible and visual alarms were not maintained and did not operate at the "Downender" area of the building. Among other methods, the following are feasible and acceptable methods to minimize the probability of injury during conveyor start-up and movement: (1) Eliminate exposure through guarding or removal of personnel from the conveyor area. (2) Repair, inspect and maintain existing warning devices in the area. Note: ANSI B11.18-1992 describes safety requirements for a variety of coil moving/processing machinery. A wide range of protective measures are presented.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to impact injuries during the operation of remote operated overhead cranes in the #2 Finishing Maintenance Roll shop and the DHCC Roll Shop. Among other methods, the following are feasible and acceptable methods to minimize the probability of injury during crane operations: (1)Provide training including hands on operational training for all crane operators in the correct procedures for; a) proper methods and equipment for rigging and hooking up the loads including proper use of lifting eye-bolts where used, b) safely moving the loads, c) standard hand signals, and d) daily visual inspections and limit switch check. (2)Ensure that only trained employees operate the radio controlled cranes. (3)Enforce regulations set forth in OSHA 29 CFR 1910.179 and ANSI B30.2.0 - 1967 incorporated by reference into the OSHA regulations. LTV STEEL COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, OR THE EQUIVALENT STANDARD 5(a)(1) GENERAL DUTY CLAUSE VIOLATION, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 103010260, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON 09/12/97.
