Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On or about July 9, 1998, at 3725 Severn Rd, Cleveland Heights, OH,
employees were exposed
to the hazards of a roof collapse resulting in serious injuries such as
broken bones, skull
fractures and lacerations while leveling and securing wood trusses
approximately 40 feet long.
The shift of approximately 50 sheets of 4 ft x 8 ft plywood roof sheating,
separated into two
piles placed along side each other on the west end of the structure,
caused the trusses to collapse
under their weight.
This hazard can be abated by ensuring all trusses are level and secure
prior to the placement of
any roofing material on top of them.
NOTE:  Per TPI, Truss Plate Institute, with regards to installing,
securing and loading of trusses
approximately 40 feet long; the installer must assure all permanent
bracing is installed prior to
the placing of any loads on them.
