29 CFR 1926.21(b)(6)(i):  Employees required to enter into confined or
enclosed spaces were
not instructed as to the nature of the hazards involved, the necessary
precautions to be taken,
and in the use of protective and emergency equipment required:
905 North Summit Street, Barberton, Ohio:  On 09-22-98, at least one
employee entered a
confined space to concrete the inside of a monopole without the benefit of
a fully developed and
implemented confined space entry program, specifically without training on
a plan of action to
conduct a timely rescue for entrants in the confined space, should an
emergency have arisen.
Note: OSHA would encourage that the company implement and enforce a
confined space entry
program in accordance with American National Standard Safety Requirements
for Confined
Spaces (ANSI)Z117.1-1995.  Elements of the program include, but are not
limited to the
following:
1) Evaluate the space as to its status; permit-required or non-permit
required;
2) Address and evaluate the  hazards involved;
3) Mark the space with a warning sign (confined space);
4) Retain appropriate retrieval equipment;
5) Formulate a written plan of action with provisions to conduct a timely
rescue for individuals
in a confined space should an emergency arise;.
6) Provide training for both the entrant and the attendant regarding
protective, testing, and
rescue equipment.
