brackets
standing in a bucket of a Kobelco, LK 500 front loader machine, were not
protected from falling, on or about 8/6/98.
Feasible and acceptable abatement methods, among others to correct this
hazard, are to ensure
that management personnel, and other employees obey the company
rules/policies as follows:
1. Ensure that employees do not stand inside the bucket to remove
material.
2. Ensure that only authorized personnel ride the material handling
equipment.
