employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were not protected from the hazard of being struck by falling load, due to
failing hoists:
a)Ashcart Rooms, Buildings 26-35:  Employees were exposed to the hazard of
a
falling load due to a failing hook and/or chain of hoist, on or about
9/1/98.
NOTE: A feasible abatement method among others is to follow the
manufacturer's instructions
to inspect the chain for wear using a gauge and to measure the upper and
lower throat openings
of the hook, on a monthly basis.y
