IN THE ALTERNATIVE

equipment having an obstructed view to the rear which did not
have a reverse signal alarm audible above the surrounding
noise level.

struck by falling material loads:
a)Waste water treatment plant - pendent controlled under-
hung electric chain hoist(Demag/ 480 vac,3 phase/2 ton
cap./ SN 791280) suspended from fixed rail was not
inspected and records maintained in accordance with
American Society of Mechanical Engineers, ASME B30.17.2.
In addition, load capacity was not conspicuously
displayed.
One feasible method to correct this condition would be the
implementation of inspection and record maintenance procedures
and training of operators in accordance with the manufacture's
guidelines and ASME document.
