NCGS 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: caught in/under/between hazards Complete inspections of the hoist was not performed at intervals defined in ANSI B30.16-1981 Chapter 16-2, article 16-2.1.1(b)(2). a)vocational building, diesel maintenance room, room 208 - no annual inspection of 2 overhead hoists. b)vocational building, truck driver training room - no annual inspection of two overhead hoists. c)Wilson building, automotive shop - no annual inspections of overhead hoist. d)maintenance building, maintenance shop - no annual inspection of overhead hoist. One feasible and acceptable means of abatement, among others, is to perform periodic inspections during the intervals indicated as stipulated in ANSI B30.16-1981, article 16-2.1.3.

The supporting structure - including trolleys, monorail, or crane, if any - was not designed to withstand the loads and forces imposed by the overhead hoist (ref. ANSI B30.16-1981, 16- 1.3.2). a)vocational building, diesel maintenance room, room 208 - support beams for 2 overhead hoists not rated for capacity. b)vocational building, truck driver training room - support beams for two overhead hoists not rated for capacity. c)Wilson building, automotive shop - support beams for overhead hoist not rated for capacity. d)maintenance building, maintenance shop - support beam for overhead hoist not rated for capacity. One feasible and acceptable abatement method, among others, to correct this hazard is to install overhead hoists with support structures which are designed to withstand the loads and forces imposed by the hoist, as stipulated by ANSI B30.16-1981, Chapter 16-1, Section 16-1.3, Article 16-1.3.2.

Hooks on overhead hoists were not equipped with latches where the application did not make the use of the latch impractical (ref. ANSI B30.16-1981, 16-1.3.9). a)vocational building, diesel maintenance room, room 208 - no safety latch on Budgit H5003 hook connected to 4 ton capacity overhead hoist. One feasible and acceptable abatement method, among others, to correct this hazard is to equip hooks with latches as stipulated by ANSI B30.16-1981, Chapter 16-1, Section 16-1.2, Article 16- 1.2.9.

NCGS 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: fall hazards. Whenever a truck was used to elevate personnel for any reason, a safety platform secured to the lifting carriage and/or forks was not utilized (ref. ANSI B56.1-1975, 513 A.1). a)maintenance building, groundskeeping storage area - lifting platform used with industrial truck was not secured to mast or forks when in use. One feasible and acceptable abatement method, among others, to correct this hazard is to secure the safety platform to the lifting carriage and/or forks, as stipulated in ANSI B56.1-1975, Part II, Chapter 513, Article 513 A.1.
