NCGS 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: caught in/under/between hazards Complete inspections of the hoist was not performed at intervals defined in ANSI B30.16-1981 Chapter 16-2, article 16-2.1.1(b)(2). a)Wake County Office Building, basement, chiller room - no inspections conducted on Dayton 1 ton overhead hoist. One feasible and acceptable means of abatement, among others, is to perform periodic inspections during the intervals indicated as stipulated in ANSI B30.16-1981, article 16-2.1.3

NCGS 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: caught in/under/between hazards. Hooks on overhead hoists were not equipped with latches where the application did not make the use of the latch impractical (ref. ANSI B30.16-1981, 16-1.2.9) a)Wake County Office Building, basement, chiller room - hook on Dayton 1 ton overhead hoist was not locking type. One feasible and acceptable abatement method, among others, to correct this hazard is to equip hooks with latches as stipulated by ANSI B30.16-1981, Chapter 16-1, Section 16-1.2, Article 16-1.2.9
