Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazard of explosion, fire and smoke inhalation from the ignition of Polyurethane foam material during the shredding, granulating and transfer process of foam material to storage hoppers. (a)Shredder Area - Shredder/Granulator Machine - Pneumatic Conveyor - Storage Hoppers Employees operating the shredder/granulator machine and pneumatic conveyor in which an adequate fire and explosion protection and prevention system was not provided to prevent the ignition and transfer of combustible foam on or about 04/27/98. ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATEMENT WOULD BE TO: 1) Follow 1990 Edition of National Fire Protection Association Standard 650 Fire standard for Pneumatic Conveying Systems for Handling Combustible Materials which states: a) Section 1.5.5 " Equipment and bins shall be provided with explosion prevention or explosion protection in accordance with either 1-5.5.1, 1-5.5.2, or 1-5.5.3. b) Section 5.1." When material is introduced into the pneumatic system through a hopper and feeding device, such hopper shall be dust-tight and designed for pressure equalization. c) Section 6-2.2 " When the product has a tendency to stick to the rotor or housing, the rotor shall be cleaned periodically to maintain good balance, the housing shall be cleaned periodically to minimize the possibility of ignition, especially when exposed to heated air. d) Section 8-2.2 A through systemic inspection for safe operation. The inspection shall include, but not limited to the following: 1. Fire and explosion protection and prevention equipment, 2. Dust control equipment, 3. Housekeeping, 4. Electrical and mechanical equipment, relief valves, and interlocks, 5. Procedures.
