Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the uncontrolled release of high pressure natural gas (i.e. a blowout), fire and/or explosion. Otto Cummings No.2 ALT well, Sec 24-T17N-R7W, Bienville Parish, LA. On or about October 24, 1998, the employer did not ensure that the completion string and unsupported length of pipe could withstand the forces were imposed upon it. Among other methods, one feasible and acceptable method to correct this hazard is to: 1. Design the completion string in such a way as to reduce the forces imposed upon it.
