General-duty citation text
Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to falls to the elevator pit below while they were climbing the frames of a tubular welded frame scaffold. Among other methods one feasible and acceptable abatement method to correct this hazard is to install hook-on ladders which would extend three feet above the frames and utilize a landing zone at the top of the structure. Job Site: Elevator shaft. Employees were exposed to fall hazards of up to thrity-five feet to the elevator pit where they were climbing the frames of a tubular welded frame scaffold to gain access to the roof.s
