Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to carbon dioxide gas in excess of the Immediately Dangerous
to Life and Health
(IDLH) concentration of 40,000 parts per million (ppm):
Thermo-Kool Walk-In Freezer: On or about June 12, 1999, an employee was
overexposed to
carbon dioxide gas (from dry-ice) in excess of the IDLH concentration of
40,000 ppm.  The
employee entered the freezer for a food order and discovered an
unresponsive worker collapsed
on the floor.  Nine 55-pound blocks of dry ice had been placed in the
broken freezer to preserve
the food.
Feasible and useful methods to correct this hazard include mechanical
ventilation, respiratory
protection, continuous visual, voice or signal monitoring, or continuous
atmospheric monitoring
of the space.
ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM.
