Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees, in that,
employees were exposed to a fall hazard from the elevated work platform.
(a) Pacific Contractors Group Inc. at 1415 Oakridge Dr., Ft. Collins, CO.,
on or before
03-23-98.  The employer did not ensure that restraining means such as body
harness(es) with
lanyard(s) were in place and used.  Additionally, the employer did not
ensure that the lift
operator was at the controls at all times, and that the operating
instructions were posted in
view of the operator.  These conditions exposed employees to the hazard of
falling from
elevation.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to instruct each employee that operates and work from the platform in
the
requirements of
ASME/ANSI B56.6-1987, Safety Standard for Rough Terrain Forklift Trucks,
Part II, 5-15
Elevating Personnel:
1. Ensure that the professional engineers operating procedures are
followed.
2. Ensure that required restraining means such as body belt with lanyards
etc., are in place
and properly used.
3. Ensure that the operating instructions are visible to the operator of
the lift, as per the
professional engineers instructions..
4. Ensure that the lift operator remains at the control as required by the
professional
engineers instruction.
Abatement Note:  Abatement certification is not required for this item.
