Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of amputation. a)On or before 6-10-98 in the press area, the two-hand control of the Denison Hydrolic Multipress #15702 was not arranged to require concurrent application by both hands for each cycle of the press. The American National Institute Standard B11.2-1982 paragraph 3.3.4(1) states: Each two-hand control station shall be arranged by design, construction, and/or separation so that concurrent application by both hands is required on each cycle of the press. Abatement note: Among other methods, one feasible and acceptable method to correct the hazard is to ensure that concurrent application by both hands is required on each cycle of the press. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter"). HAZARD: Amputation. The operating pressure of the press is sufficient to cause an amputation of the fingers or the hand if these body parts were placed into the die area. HAZARD MUST BE RECOGNIZED: ANSI B11.2-1982 paragraph 3.3.4(1) requires that the two hand control be constructed so that concurrent application by both hands is required on each cycle of the press. THE HAZARD WAS CAUSING OR WAS LIKELY TO CAUSE DEATH OR SERIOUS PHYSICAL HARM: During production work, the employees hands are placed into the dies to load stock into the die, and to retrieve the finished product out of the die. THE HAZARD CAN BE CORRECTED BY A FEASIBLE AND USEFUL METHOD: Aproppriate two hand controls are widely used in this industry and readily available from many sources.

The American National Institute Standard B11.2-1982 paragraph 3.8.1.1 states: The press shall show the maximum tonnage as determined by pressure settings in accordance with press design. Where applicable, counterbalance pressure settings for die weights shall be affixed to the press. Abatement note: Among other methods, one feasible and acceptable method to correct the hazard is to ensure that the press is labled with the maximum tonnage. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter"). HAZARD: Laceration. If the maximum rated tonnage is not known/labeled, damage can occur to the press or die set, causing lacerations to the operator. HAZARD MUST BE RECOGNIZED: ANSI B11.2-1982 paragraph 3.8.1.1 requires that the press be marked with the maximum tonnage. The American National Institute Standard B11.2-1982 paragraph 6.2(1) states: All dies shall be: Identified with the tonnage and, if necessary, stroke requirements; and/or these characteristics shall be recorded and available to the die setter. Abatement note: Among other methods, one feasible and acceptable method to correct the hazard is to ensure that the dies are labeled with the tonnage and stroke. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter"). HAZARD: Broken bones. If the maximum rated tonnage and stroke are not known/labeled, damage can occur to the press or die set, causing broken bones to the operator or die setter. HAZARD MUST BE RECOGNIZED: ANSI B11.2-1982 paragraph 6.2(1) requires that the dies be marked with the maximum tonnage and stroke or avilable to the die setter. THE HAZARD WAS CAUSING OR WAS LIKELY TO CAUSE DEATH OR SERIOUS PHYSICAL HARM: If the maximum rated tonnage and stroke is not known/labeled, damage can occur to the press or die set, causing broken bones to the operator or die setter. THE HAZARD CAN BE CORRECTED BY A FEASIBLE AND USEFUL METHOD: The manufacturer can assist the employer in determining the maximum tonnage and stroke, and the employer can label this information on the dies.

The American National Institute Standard B11.2-1982 paragraph 6.3 states: Die setting and/or repair. The employer shall provide and enforce the use of either safety blocks interlocked with the press control circuit or full-tonnage blocks to prevent the closing of the dies when access is required to the die area. Where the size oor design of a particular press precludes the use of these methods,other reliable means of holding the ram or slide in the up position shall be provided. Abatement note: Among other methods, one feasible and acceptable method to correct the hazard is to ensure that safety blocks are used to prevent the closing of the dies when access is required to the die area.. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter"). HAZARD: Broken bones or death. When the diesetter is in the point of operation, the press could cycle and cause broken bones or death. HAZARD MUST BE RECOGNIZED: ANSI B11.2-1982 paragraph 6.3 requires the use of safety blocks for die setting and repair.
