states: Each two-hand control station shall be arranged by design,
construction, and/or
separation so that concurrent application by both hands is required on
each cycle of the
press.
Abatement note:  Among other methods, one feasible and acceptable method to
correct the hazard is to ensure that concurrent application by both hands
is
required on each cycle of the press.
Abatement Note:  Abatement certification and documentation are required for
this item (See enclosed "Sample Abatement-Certification Letter").
HAZARD: Amputation.  The operating pressure of the press is sufficient to
cause an
amputation of the fingers or the hand if these body parts were placed into
the die area.
HAZARD MUST BE RECOGNIZED:  ANSI B11.2-1982 paragraph 3.3.4(1) requires
that
the two hand control be constructed so that concurrent application by both
hands is required
on each cycle of the press.
THE HAZARD WAS CAUSING OR WAS LIKELY TO CAUSE DEATH OR SERIOUS
PHYSICAL HARM:  During production work, the employees hands are placed
into the dies
