that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to fall hazards from an elevated personnel platform:
(a)Carlson Custom Framing at 807 Mountain Avenue (Berthoud National Bank)
Berthoud, CO 80513:  On and before 04-07-99, the employer did not ensure
that a wooden, job-made personnel platform used in conjunction with a rough
terrain forklift (LULL #8448) was properly designed, constructed, utilized
and
maintained.  Condition exposed employees to an approximate 27 foot fall
hazard.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to design, construct, utilize and maintain the
personnel
platform in accordance with ASME/ANSI B56.6-1992, Safety Standard for
Rough Terrain Forklift Trucks.
